Acentra Health is actively hiring Clinical Evaluators to join our growing team in Los Angeles, California. We currently have three PRN openings available in the Los Angeles area.

 

Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Los Angeles, California. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Los Angeles.

As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field

Job Responsibilities

  • Conduct thorough and accurate PASRR Level II assessments and evaluations by reviewing relevant records, gathering input from collateral sources, and evaluating the individual’s physical, behavioral, and psychological functioning.
  • Responsible for conducting Preadmission Screening and Resident Review assessments and developing informed level-of-care recommendations, and determining the most appropriate next level of care for those who may require long-term care placement.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation
  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Knowledge of standards of practice related to Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Ensure compliance with established workflows, including re-evaluations of records based on psychiatric input.
  • Understand, and comply with all corporate policies, including those related to HIPAA and its Privacy and Security Rules.
Qualifications:

Required Qualifications, Knowledge, and Experience

  • Must possess an active, unrestricted license in the State of California in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) and Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 - 3 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.

  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.

  • Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.

  • Demonstrated expertise in assessing and analyzing an individual’s psychological, emotional, behavioral, and physical functioning. .
  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Experience in medical record abstraction and clinical data review.

Preferred Qualifications

  • Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including compliance with state and federal regulations.
  • Knowledge of Federal Regulation 42 CFR, Chapter 483, related to long-term care and resident assessment requirements.
  • Familiarity with mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions commonly requiring long-term care services and supports.
  • Understanding of psychiatric disorders as defined in the current edition of the DSM, in accordance with applicable state guidelines.

  • Detail-oriented with strong organizational skills and able to prioritize and complete tasks efficiently within set timelines
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
  • Previous experience working with the geriatric population.

Compensation

The compensation for this role is $40.00 - $42.00 per hour base

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
